Before you start filling out this form, please make sure you have the required information regarding club officers available.

You will also be required to attach and submit the Club Membership Form and the Club Constitution.

This form should be filled out with the most updated information for the 2024-2025 academic year.

Once you've submitted all mandatory forms, you will receive an e-mail from the Office of Campus Life and Student Engagement confirming your active standing for the club term or next steps on becoming chartered.

If you need to make any changes during a club term (i.e. officer change), please e-mail campus.life@canyons.edu with the list of changes.

ESTABLISHED/RECHARTERING CLUBS:
This form must be filled out every club term to keep its active standing in the Inter Club Council. Established/rechartering clubs must submit all chartering forms by November 15, 2024 at 11:59pm. Failure to fill out this and other mandatory forms may cause your club to go on probation or inactive status.

NEW CLUBS:
This form must be filled out before you are able to become chartered.

To become chartered as a NEW club for 2024-2025, this form must be submitted by January 31, 2025 at 11:59pm.
