Campus Life & Student Engagement Club Material Reservation Form

I agree to be financially responsible for the equipment listed below while in my possession.  I understand that it is my responsibility to ensure all components of the equipment are included, at the time of pickup and upon return.  I take full responsibility for the equipment’s transportation, setup, and use.  Furthermore, I understand and agree that it is my responsibility to return the equipment to one of the staff members at Campus Life & Student Engagement who will complete all necessary procedures to check the equipment back in.  

By submitting the form, I acknowledge that all material requests made to Campus Life & Student Engagement are for student events and activities only.  Additionally, I agree to abide by and conform to all rules concerning District property and will reimburse Campus Life & Student Engagement for any costs due to violation of said rules and regulations.  I may lose the right to reserve the machines if not brought back in the same condition as it was during check-out.

Failure to comply with this agreement may result in a hold placed on your Records at College of the Canyons.

Submit reservation at least 2 weeks prior to the event date.  All food supplies used with any of the machines must be approved by Campus Life & Student Engagement.
